Job Description:
Good Samaritan Health Center is seeking a Licensed Social Worker
dedicated to providing direct and effective social work services
with a focus on holistic health and well-being. Our Licensed Social
Worker will play a critical role in our mission to deliver
comprehensive healthcare across underserved communities, ensuring
that social health barriers are addressed to improve patient health
outcomes. This role involves patient assessment, crisis
intervention, care coordination, and community outreach. The
successful candidate will advocate for patient needs and work
towards broad community health goals in a supportive and
resource-rich environment. Conduct comprehensive assessments of
patient social, emotional, and environmental needs. Develop and
implement care plans that address identified needs, including
mental health services and community resources. Participate in
interdisciplinary healthcare teams to discuss care coordination and
intervention strategies. Advocate for patients and communities
within the healthcare system and external agencies to access
necessary services. Facilitate support groups and educational
workshops on health management and social issues. Maintain detailed
records of patient interactions, services provided, and outcomes.
Strong interpersonal and communication skills Ability to handle
high-stress situations and crisis intervention effectively
Proficient in case management software and patient care systems
Knowledge of community health resources and referral processes
Empathy towards diverse patient populations and challenges Medical

Current and valid state license as a Licensed Clinical Social
Worker (LCSW) or Certified Social Worker (CSW) depending on state
regulations.
